Applicant should complete all sections of the permit form marked with a blue asterisk (*). Bring the form to the office of the City Engineer for approval. A member of the Engineering Division staff will make a site inspection to determine what, if any, conditions will be attached to the permit and to determine pipe size, if applicable.

The permit requires the signature of the City Engineer or his designee and is invalid without that signature and unless a copy of the approved permit is on file in the office of the City Engineer.
