Applicant must complete all sections of the permit form marked with a blue asterisk (*). The applicant must bring the permit form to the office of the City Engineer for approval and payment of the permit fee. The bond and insurance documents required by the permit must be attached when the form is submitted for approval. Be sure that the City of Bristol, Virginia is named as an Additional Insured on the Certificate of Insurance.

The permit is not valid without the signature of the City Engineer or his designee, until the fee has been paid and a copy of the approved permit is on file in the office of the City Engineer.
